Path Assist is a free program to help people find the resources they need.

If you are in need of housing, food, employment, education, transportation, healthcare, childcare, or financial assistance, we may be able to help.


Find resources

Whether it’s through our online resources kit or one of our Community Health Navigators, you will be connected with tools to help you find what you need to meet your goals.


Work with a Community Health Navigator

Depending on your needs, you may work with a local Community Health Navigator (CHN).

This person:

Knows the resources in your area really well.

Can talk to you about barriers you might be experiencing when seeking resources like food, transportation, or childcare.

Can help you set goals and make a plan to meet them.


What Path Assist cannot do…

Path Assist cannot give money or resources directly to clients.
However, we can do our best to connect you to the services that can help you get what you need.
