What To Expect?

Working with a Community Health Navigator

A Community Health Navigator knows the resources in the area really well and is skilled at navigating them. They are also trained to listen closely to clients and work with them to set goals and make plans for reaching them. When working with a CHN you are always in the driver’s seat.

Week 1: Intake Session

• Learn about YOU!
• Review needs and set goals
• Make an action plan and talk about potential barriers
• Coordinate care and discuss resources
• Assess confidence in completing week’s task
• Schedule next check in call

Each Week: Follow up Sessions

• Review progress
• Update needs and set goals
• Make an action plan and talk out potential barriers
• Coordinate care and discuss resources
• Coach and skill build
• Assess confidence in completing week’s task
• Schedule next check in call
